Position: Senior Health Liaison Worker
Location: London Road, SM4 5DX
Start Date: ASAP
Contract Duration: 3+ Months
Working Hours: 35 hours per week (09:00 – 17:00)
Pay Rate: £ 28.60 per hour

Job Responsibilities
Act as a Senior Qualified Social Worker within adult social care services
Support health and social care integration and liaise with partner agencies
Undertake assessments in line with relevant legislation and local policies
Apply safeguarding procedures to protect vulnerable adults
Work in accordance with Care Act duties and mental health legislation
Promote person-centred approaches, independence, and wellbeing
Contribute to care planning, reviews, and risk assessments
Maintain accurate and timely case records and reports
Work collaboratively within a multidisciplinary team
Person Specification
Must-Have Requirements
Social Work England registration (or equivalent valid professional registration)
Proven knowledge of relevant legislation, including:
Care Act
Mental Capacity Act
Mental Health Act
NHS and Community Care Act 1990
Disability Discrimination Act 1995
National Assistance Act 1948
Understanding of adult safeguarding frameworks and Pan London Safeguarding Strategy
Eligibility to work in the UK
Valid Enhanced DBS (or willingness to obtain)
Ability to manage complex adult social care cases
Nice-to-Have Requirements
Experience working in adult social care within a local authority setting
Experience in health and social care liaison roles
Familiarity with London Borough working practices, particularly Merton
Experience working in multi-agency or integrated care environments
Strong report writing and case management skills
